e-Consult
e-Consult is available Monday to Friday (excluding Bank Holidays)
Prior to ringing the Practice, please complete the eConsult easy-to-use questionnaire for:
- an appointment.
- requesting an ongoing medical certificate.
- submitting your medication review.
The form will automatically send to the Practice. It is the best way to consult with a clinician.
Econsult for total Triage will be available Monday - Friday between 7am - 2pm. Emergencies only can still contact the surgery after 2pm.
Stifford Clays Medical Practice would like you to use eConsult as the preferred method when booking an appointment, this is not an Emergency service.
A GP will review your eConsult request and a care coordinator will contact you to book an appointment if appropriate. The GP may also contact you if further information is needed.

Appointment System
The doctors hold surgeries in the mornings and afternoons each day. We endeavour to have at least two doctors on duty on the later surgery to cater for those patients who work. One doctor is appointed ‘duty doctor’ each day and he/she will see all emergency and extreme cases on the day.
Please feel free to express a preferred practitioner when making your appointment, the doctors would encourage you to stay with one clinician for any particular health issue or episode of care,
If eConsult is not an option for you, an appointment can be made either by calling into the surgery during opening hours, or telephoning 01375 800450 Monday to Friday between 9am and 2pm Emergancies only can still contact the surgery after 2pm. At busy times, especially in the mornings, there may be a delay before your call is answered due to the telephone lines all being engaged. Please be patient and try again later. Our Care Navigators will ask for a brief description of your condition and complete an eLite request on your behalf for the GP to assess, it is intended to help direct you to be seen by the most appropriate person.
Chaperones
If you are to have an intimate examination whether in the surgery or over the telephone, please feel free to ask for a chaperone.

Evening and Weekend Appointments
We want to provide the right services at a time which is convenient for you. We’ve listened to feedback from people in our local area, and we are making changes to our out-of-hours appointments to deliver better services for you.
The new way of delivering out-of-hours appointments will give you:
- Access to more healthcare professionals. This new service will be delivered by professionals including GPs, nurses, Paramedics, and clinical pharmacists.
- More choice about the type of appointment you want. These include a mixture of in-person face-to-face appointments, and remote appointments like telephone, video, or online appointments.
- Make it more convenient for you to travel to out-of-hours appointments. These appointments are being planned and delivered locally, so it is likely they will available closer to your home.
Out-of-hours appointments are available from 07:00 - 08:00 & 18:30 to 20:00 on weekdays, and from 09:00 to 17:00 on Saturdays, this services is delivered locally by Primary Care Networks (PCN).
An out-of-hours appointment might not be at your regular GP surgery. When you book an appointment, you will be told where the appointment will be.
These appointments will not be available as walk-in appointments. You will need to book them in advance.
To book an out-of-hours appointment, you should contact your regular GP practice in the usual way. They will advise you on the appointments which are available, and which healthcare professional is the right person to help you.
If you need medical help now but it's not an emergency, you can also go to 111 online or call 111.
Home Visits
Home visits take two or three times as long as a surgery consultation so please only request a visit when absolutely necessary.
You may be visited at home by the surgery paramedic or a community nurse if you are referred to them by your GP. You could also be visited at home by a health visitor if you have recently had a baby or if you are newly registered with a GP and have a child under five years.
Cancellations
If you cannot keep an appointment or no longer need to see the doctor, please let us know as soon as possible so that the appointment can be offered to someone else. Please call us on 01375 800450 Option 2 for all cancellations.
DNA – Did Not Attend
A ‘DNA’ (Did Not Attend) is an occasion on which a patient does not attend a booked appointment with a member of the practice team without contacting the surgery to cancel prior to the appointment time.
The loss of this clinical time has a detrimental effect on the service that we provide to patients, and we wish to keep this to a minimum. We feel that it is reasonable to ask patients to let us know in advance if they do not intend or are not able to keep their appointment. Patients who are experiencing ongoing difficulties in keeping their appointments should discuss this with the surgery.
The surgery reserves the right to remove patients from the practice list if they DNA three appointments within a 12 month period.
When a patient does not attend an appointment without giving advance notice the doctor will code the missed appointment on the patient record. This generates a DNA letter that is sent to the patient.
If you repeatedly fail to attend for appointments you may be removed from the Practice list and have to find an alternative GP practice.
Late Arrival for Appointment
- Up to 5 minutes late – patient will be seen.
- 5 to 15 minutes late – doctor with whom appointment originally made will see patient at the end of surgery.
- Over 15 minutes late – patient must re-book or if appropriate he/she may be booked in as an emergency in the next available emergency slot, this may not necessarily be with the doctor with whom appointment originally booked.

- If there’s a specific doctor, nurse or other health professional you would prefer to respond
- If you would prefer to consult with the doctor or nurse by phone, face-to-face, by video call or by text or email
- If you need an interpreter
- If you have any other access or communication needs
Text Reminder Service
We have a texting service which allows you to receive confirmation and reminders about your appointments.
To have this service you will need to register by completing a consent form.
Please remember to update your contact details with us when you change address, telephone numbers and email address.
Travel vaccinations
If you're planning to travel outside the UK you may need to be vaccinated against some of the serious diseases found in other parts of the world.
Training Practice
We have been a GP Training Practice since 2004 but have trained Nurses for far longer. We train all levels of Doctors in conjunction with the training programmes at Basildon University Hospital. All the Partners and Staff are very committed to providing high-quality training. Dr R Rasool is the GP Trainer & Educational Supervisor. Dr R Rasool is an Associate Trainer & GP Tutor for South West Essex. Dr M Chandran is the GP training lead for junior Doctors.
As a consequence of this you may find a junior Doctor or a Nurse in training sitting in on your consultation with one of the clinicians. If this makes you uncomfortable feel free to say so and they will leave the room.
Later in their training within the practice they will undertake consultations on their own, and you will be informed that your appointment is with one of the trainees, but please be assured that all these consultations are reviewed by the supervising Doctor ( for doctors) or Nurse ( for nurses ) each day at the end of each surgery session.
