Register as a New Patient

Please click on the above map to view our current catchment area.

When requested this will be completed by our Admin team and an appointment made for a ‘new patient interview if needed.  A patient has the right to express a preference to receive medical services from a particular doctor within the practice; this should be made in writing to the Practice Manager.  However, it should be noted that such requests could mean waiting longer than average for an appointment and cannot be made in instances where immediate or emergency treatment is required.

  1. Newly registered patients are invited to have an initial consultation with a medical practitioner within six months of registration.
  2. Patients between the ages of 16 and 74 years who have not been seen by a medical practitioner of this practice within the period of three years prior to the date of the request may also seek a consultation.
  3. Patients aged 75 and over can have a consultation if he/she has not participated in a consultation within the period of twelve months prior to the date of the request.

Consultations under 1, 2 & 3 above may take place in the patient’s home where, within the reasonable opinion of the medical practitioner, it would be inappropriate, as a result of the patient’s condition, for him/her to attend at the practice’s premises.

Online 'Pre-Registration' With The Practice

If you wish to pre-register click on the links below to open the forms. When you have completed all the details, click on the "Submit" button to mail your form to us. You must then visit the surgery to confirm that the details are correct and show ID and confirmation of your address before you are officially registered.

Temporary Registrations

If you are ill while away from home or if you are not registered with a doctor but need to see one you can receive emergency treatment from the local GP practice for 14 days. After 14 days you will need to register as a temporary or permanent patient.

You can be registered as a temporary patient for up to three months. This will allow you to be on the local practice list and still remain a patient of your permanent GP. After three months you will have to re-register as a temporary patient or permanently register with that practice.

To register as a temporary patient simply contact the local practice you wish to use. Practices do not have to accept you as a temporary patient although they do have an obligation to offer emergency treatment. You cannot register as a temporary patient at a practice in the town or area where you are already registered.

Non English Speakers

These fact sheets have been written to explain the role of UK health services, the National Health Service (NHS), to newly-arrived individuals seeking asylum. They cover issues such as the role of GPs, their function as gatekeepers to the health services, how to register and how to access emergency services.

Special care has been taken to ensure that information is given in clear language, and the content and style has been tested with user groups.
