Should you wish to access online services to view your records please visit the surgery to complete a request form.  You will need to provide photographic proof of ID to obtain your username and password as medical records are protected under GDPR regulations.

If you are applying to view someone else's online record you will need proof of power of attorney or proof of proxy access.  Parents/Guardians will be able to access children's records up to the age of 13.
